Person Centered Approaches to Healthcare Decision Making – Spring 2023

Designed for people working in the field of disabilities and aging who typically have responsibility for service planning (care coordinators, case managers, social workers), this is a facilitator course that helps planners feel better prepared to support people with disabilities identify their healthcare decision maker, and home in on decisions that matter to them. The course is highly interactive, provides myriad resources for planning tools and planning guides, offers participants a chance to practice hard conversations, and provides both clinical and empathetic content related to advance care planning, as well as care at end of life.
